This place has been horrible since the day my family member has been there. The food is horrible and baby portions. I seen a roach in the bathroom. Nurses are not friendly unless you are praising them. Medicine is not given correctly and doses are missed. I would not let my dog stay in this facility. This is unacceptable for health care. I am understanding more and more why the low star review. Never in my life have I seen nurses and other staff treat not only the patient but the family the way they have. I will be making some phone calls about this facility!!!!!

Consistently inconsistent! While there are some super star employees such as Teresa, Derek, Don, Renae, Lou, Daisy, CeeCee and all the physical therapists, there are employees who spend their time on their phone, are sarcastic and rough with some of the residents, and who argue with other employees in front of the residents. There are roaches; ridiculous rules for adults, and poor nutritional meal planning. The weekend employee schedule is sparse and they are actually serving leftovers twice today. One of the employees ordered me to stop assisting my roommate and then told her that she would be punished if she didn’t do what she was told. Medications are not dispensed correctly unless the resident keeps track of prescriptions and dosages and insists they follow orders. Showers, clean sheets, trash removal, and supplies are inconsistently provided. The level of resident care is below average despite the strong performance of the super stars. The entire staff needs to be retrained in terms of their roles and responsibilities and they need to be reminded. they chose to work in health care to HELP people.

I am writing this on behalf of my mother who stayed here. She had a very pleasant experience with Mrs. Teresa. She said that she was always friendly, willing to help with anything she needed, would go the extra mile for everything, and always had a smile on her face. Even in the brief moment that I had met her she greeted me with kindness and offered any help she could. She truly is a genuine caring person. Thank you Mrs. Teresa.

Hello, I'm currently A Resident of this facility (Avaita At The Garden's). And during My stay I've witnessed some ups and downs, some good and bad. BUT, I think that is called LIFE. Not everyone at One time can have what they want their way. The CNA'S Are the Working forces of the Resident. So I don't know exactly how many currently Occupy the rooms. But when one have a need in one place, there's another have a need in other places. When We have The Chance to Understand this Then Everything can fall into place with Less Tension. On to another subject what I hear quite often is the Kitchen Staff on the lack of Preparation of the meals how it's flavored and portioned. I admit I was guilty in this area also. Once again, We are NOT Home, things will taste different than what we have been Use To. THOSE who are in There right should Use Optimism Side. Someone Somewhere, don't have Nothing to EAT, So be Thankful. Besides That, The Nursing Staff is Pleasant, Professional, And Often Prompt. Thanks, ACQUENETTA BRANCH ✌🏾

GNR has great support and teamwork the nurses and aids go above and beyond for the residents our unit manger Chelsea makes the residents and staff feel at home she decorates for every holiday and special occasion.

My mother was admitted here last year. The team is seemingly understaffed and the facility is filthy, which I could only imagine is a management problem. The one upside are the nurses and providers. With my interactions with them, they are receptive and give an earnest effort to work with what they have. Despite that, I cant give this facility a good review because of all the other factors.

This was the most disgusting, ghetto, roach infested place I ever seen. This place really needs to close the doors and gut that place out. I was there for rehabilitation, my first night there I killed 2 roaches and a ant smh I was like oh wow I couldn’t even sleep that night bc I was scared of something crawling on me in my sleep! The CNA I had that night was phenomenal she tried to make me as comfortable as possible! (Wish I remembered her name) Now some of them was very rude would come in your room no hey good morning or nothing or act as if you was a bother to them! Now every nurse I had was very nice especially night shift! Don’t push the call button and expect someone to come bc it takes them 20-30 minutes to get there and remind you there just sitting in the hallway or gone outside smh my daughter came in to visit me and the light was on bc I couldn’t move at the time and needed to be changed and she witness me having to sit there in urine for 20 minutes until she just help me herself! The food they need to just fire every one that work in the kitchen that food is hideous! They don’t care what they feed you and it be cold as hell when you get it (thank God I had family that b

A shocking level of Neglect, Non-Response to Call Button, Pain Medication hours late, No-one changing my bedsheets, No shower, and No continued medical care are just a few of the issues I, a 60 year old disabled ♿️woman have experienced since being transported from TMH to this facility. As I write this review the State of Florida Agency for Healthcare Administration (AHCA) is here this week conducting an investigation on complaints. This facility has cockroachs, antiquated furnishings, and, antiquated and mostly broken rehabilitation equipment. Search for reviews on Facebook as well and you'll find mine. There's also, what appears to be, bedbug bites 😱 on my arms now. There's obvious results of roof leaks to be seen and the roof may be compromised. I went from help getting in and out of a bed, a Purewick, and daily examinations of my body wounds at TMH to no help here at all, limping my way to the bathroom with only the assistance of a walker that I had to ask for upon getting here and realizing my situation. Again, to get help by pushing the call button cannot be counted on and they do not have an intercom system in the rooms. Food, well I'm not a picky person but, although
